资格要求：1.如果投标人为中华人民共和国境内注册公司，需提供有效的营业执照（具备独立法人资格）、一般纳税人证明/税务登记证、组织机构代码证复印件或三合一营业执照，投标时提供原件扫描件（原件备查）。如果投标人为分公司的，应具有有效的证照合一的营业执照和总公司合法授权书。总公司与分支机构只可一家参与投标，同时参与投标视为投标无效。如果投标人为事业单位的，投标人应经全国各级事业单位登记管理机关核准登记或者备案，具有合法有效的事业单位法人证书。如出现单位负责人为同一人或者存在控股、管理关系的情况，相关投标均无效，招标人随时有权作出拒绝投标、取消投标资格、取消授标、不签订合同或在合同签订后终止合同等决定，相关投标人须无条件接受，且由此造成的任何损失由投标人承担。如果投标人为境外注册公司，需提供有效的公司登记注册证明。2.投标人应为本次投标产品的制造商。本次招标不接受代理商、贸易商投标。本次接受所投货物的制造商全资的销售公司或控股的销售公司，以及接受所投货物制造商的母公司参与投标，此类公司视同为制造商，投标时需提供相应的证明文件。同一制造商仅允许一家投标人参与投标，否则相关的所有投标将被否决。3.投标人所投产品的制造商须具备有效的GB/T19001（ISO9001）质量体系认证证书或API Q1证书，并可在中国国家认证认可监督管理委员会网站(http://www.cnca.gov.cn/)核实。如所投货物的制造商提供的质量管理体系认证证书是国外第三方认证机构颁发的，则网站查询不适用。Qualification Requirements:1.If the bidder is a registered company in the People's Republic of China, it is necessary to provide a valid business license (with independent legal personality), general taxpayer certificate/tax registration certificate, copy of organization code certificate or three-in-one business license, and provide a scanned copy of the original (original for reference) when bidding. Checking is not applicable to certificates issued by foreign third-party certificate authority.业绩要求：（开标时需进行信息公开）1.2020年1月1日至投标截止日（以合同或订单签署时间为准），投标人所投产品的制造商应具有1个合同的潜水设备（或系统）的业绩。2.投标人须按规定格式提交业绩表，并提交相关业绩证明文件。业绩证明文件包括但不限于：1）合同扫描件和2）到货验收材料。投标人所提交的业绩证明文件必须至少体现以下内容：合同签署时间、合同签署页（应有双方签署或盖章）、制造商名称、货物名称及到货验收材料（至少包含：货物验收单或结算清单或对应的结算发票（如有清单须提供，须体现发票号；发票需包含合同号或订单号或货物名称或买方卖方信息，且与合同或订单一致）或买方（或最终用户）接收证明或调试验收证明或提单（含箱单和发票）或报关单或买方（或最终用户）签署的SAT（现场接收试验）报告。若业绩为订单，需提供对应的协议扫描件、订单扫描件及订单相应的到货验收材料。货物验收单或结算清单至少应有合同甲方签字或盖章。3.未提交业绩证明文件，或所提供的业绩证明文件无法体现满足上述业绩要求的，均视为无效业绩。Performance requirements(Performance track record will be released to all bidders during bid opening): 1.From 1 January 2020 to the deadline for bidding (subject to the time of signing the contract or order), the manufacturer of the product invested in by the bidder shall have the performance of a Diving Equipment/System in one contract. 2.The bidder shall submit the performance statement in accordance with the prescribed format and submit the relevant performance certification documents.
