资格要求：1、投标人具有合法有效的企业法人营业执照、税务登记证及组织机构代码证或证照合一的营业执照，投标时需提供原件扫描件（原件备查）。投标人为事业单位的，应具有合法有效的事业单位法人证书，投标时需提供原件扫描件（原件备查）。投标人为分公司的，应具有合法有效的营业执照和上级法人单位授权书，分公司与上级法人单位只可一家参与投标，同时参与投标的，投标均无效。 如果投标人为境外注册公司，需提供有效的公司登记注册证明。2、投标人应为本次投标产品的制造商。本次招标不接受代理商、贸易商投标。本次接受所投货物的制造商全资的销售公司或控股的销售公司，以及接受所投货物制造商的母公司参与投标，此类公司视同为制造商，投标时需提供相应的证明文件。同一制造商仅允许一家投标人参与投标，否则相关的所有投标将被否决。3、如出现不同投标人的负责人为同一人或存在控股、管理关系的情况，相关投标均无效，招标人随时有权做出拒绝投标、取消投标资格、取消授标、不签订合同或在合同签订后终止合同等决定，相关投标人须无条件接受，且由此做出的任何损失由对应投标人承担。4、投标人所投产品的制造商投标时需提供合格有效的GB/T19001（ISO9001）质量体系认证证书（应包含生产或制造），并可在中国国家认证认可监督管理委员会网站(http://www.cnca.gov.cn/)核实。如果有国家相关部门发布的最新体系标准，以最新体系标准为准。投标时需提供原件扫描件（原件备查）。注：如所投货物的制造商提供的质量体系认证证书是外国第三方认证机构颁发的，则网站查询不适用。Qualification Requirements:1. The bidder shall be the manufacturer of the bid product. This tender does not accept agents, traders bid. The wholly-owned or majority-owned sales company of the manufacturer of the goods accepted this time, and the parent company of the manufacturer of the goods accepted to participate in the bidding, such companies shall be regarded as manufacturers, and the corresponding supporting documents shall be provided when bidding. Only one bidder from the same manufacturer is allowed to participate in the bid, otherwise all relevant bids will be rejected.3. If the person in charge of different bidders is the same person or there is a holding or management relationship, the relevant bid shall be invalid, and the tenderer has the right to make decisions at any time such as rejecting the bid, disqualifying the bid, canceling the award of the bid, not signing the contract or terminate the contract after the signing of the contract, etc. The relevant bidder shall unconditionally accept the decision, and any losses arising therefrom shall be borne by the corresponding bidder.4. Note: If the quality system certification certificate provided by the manufacturer of the goods is issued by a foreign third party certification body, the website query is not applicable.业绩要求：（开标时需进行信息公开）1、2018年1月1日至投标截止日（以合同签署时间为准），投标人所投产品的制造商应具有至少1个1500磅插入式过球指示器的供货业绩，测量介质不限。（1）投标人须按规定格式提交业绩表，并提交相关业绩证明文件。业绩证明文件至少包括：1）销售合同复印件（内容至少包括：合同首页、签字盖章页、签署时间、制造商名称、货物名称、磅级1500磅、形式为插入式过球指示器和2）到货验收材料（调试验收报告或买方接收证明或结算证明）。未提交业绩证明文件，或所提供的业绩证明文件无法体现满足上述业绩要求的，均视为无效业绩。Performance requirements(Performance track record will be released to all bidders during bid opening): 1. From January 1, 2018 to the bid deadline (subject to the contract signing date), the manufacturer of the bidder's bid shall have a supply record of at least one 1500 lb plug-in ball indicator with no limitation on the measuring medium. Failure to submit performance documentation, or the performance documentation provided cannot reflect that the above performance requirements are met, shall be regarded as invalid performance.
